What we expect from you:
- Arrive on time and be ready for appointments, making sure you have completed any necessary tests or bloodwork
- Always bring your medications and blood sugar monitoring devices to your appointments
- If you are unable to make an appointment, let us know as early as possible
- Let us know if there are any changes to your health
- Share your contact information and update it either by phone, email, or in person
- Treat us with respect and courtesy
- Continue to see your family doctor for care not related to kidney disease
- Ask questions if there is something you are not sure about
We provide a range of services for transplant candidates, recipients and living donors, including:
- Transplant candidate assessment
- Living donor candidate assessment, including out-of-country donors when possible
- Post-transplant follow-up care with a multidisciplinary team
- Health information online and in print, group and individual education, and a quarterly newsletter
- Opportunities to participate in clinical research
- Opportunities for patients and families to provide feedback about our clinic with the Patient and Family Advisory Council
